- UK region
- Approximate date of incident
- Category of incident
- Whether you are on a sponsored visa
- Whether there was employer retaliation or injury
- An optional, free-text description (do not include your name)
What we do NOT collect
- Your name, address, or contact details
- Your employer's name
- Witness names or details
- Any browser cookies or stored data
